Community Complaint Form

  1. Welcome to the City of San Mateo's Online Community Complaint Form

    We only accept complaints on issues for the the City of San Mateo, not the County of San Mateo or other cities.

    If you live, work, or are visiting San Mateo and notice one of the problems listed below, please use this form to notify us.

    Simply select the type of problem you are reporting from the list below and complete the information boxes. When you hit the "Send form to city" button your report will automatically be sent to the appropriate City department for action.

    Thank you for taking the time to assist us in maintaining our City.

    Reports will be processed 8:00am - 5:00pm PST, Monday through Friday.

    NOTE: Reports received on City Holidays will be processed on the next business day.
